Accidental feathering caused a plane crash in Nepal that claimed 72 lives: report

 

According to a government-appointed investigative report, the pilots of a Yeti Airlines plane accidentally feathered the propellers, which resulted in a crash that claimed 72 lives.


A loss of thrust brought on by the feathering resulted in an aerodynamic stall.


On January 15, 2023, just before it was scheduled to land in the popular tourist destination of Pokhara, the Yeti Airlines ATR 72 aircraft crashed. It was the deadliest plane crash to occur in Nepal in thirty years.


Fifty-one foreign nationals, four crew members, and two infants were among the 72 victims of the flight. Not a single person survived.


The report states that neither pilot's 72-hour history suggested that fatigue or medical problems had anything to do with the deadly error. According to the report, "both pilots were reported to have eaten routine foods, gone to bed, and awoke at routine hours." 


It was discovered that the twin-engine aircraft had received proper maintenance, with no maintenance work past due. 


Additionally, the report indicated that it was concluded that the accidental shifting of both condition levers to the feathered position during flight was the most likely cause of the accident. Due to the feathering of both propellers, there was a loss of thrust, which caused the aircraft to aerodynamically stall and crash into the ground.


The report stated that "the flight crew failed to identify the problem and take corrective action following the unintentional feathering of both engine propellers," despite warnings from the Crew Alerting Panel.


The report also noted that non-compliance with standard operating procedures and a high workload and stress were human factors that contributed to the accident.
